Arteria mesenterica superiore

L'arteria mesenterica superiore è considerata, sul piano dello sviluppo, parte del complesso celiaco e ne consegue che le variazioni della mesenterica superiore sono correlate, in larga misura, a quelle del tronco celiaco.

Le variazioni di questa arteria sono molto comuni e numerose.

L'arteria può essere doppia e originare dall'aorta come due tronchi.

Occasionalmente fornisce rami accessori al fegato, allo stomaco, al pancreas, alla milza e alla colecisti.

L'arteria può dare origine a rami che di solito derivano da altre fonti: l'arteria epatica o il suo ramo destro o sinistro, la cistica, la gastroduodenale o il suo ramo epiploico destro, la gastrica sinistra, un'arteria colica accessoria (arteria colica sinistra superiore) o un ramo pancreatico di maggior calibro. Koizumi e Horiguchi (1990) hanno riscontrato un'arteria colica accessoria che originava dall'arteria mesenterica superiore più prossimalmente rispetto alla prima arteria digiunale e raggiungeva l'arteria marginale a livello della flessura splenica; è stata riscontrata in 32 soggetti su 65 (49,2%). Gli autori, rilevando il carattere costante di questa arteria priva di denominazione, hanno proposto il nome «arteria colica sinistra superiore (arteria colica sinistra superior; CSS)». Gli autori hanno inoltre riscontrato un ulteriore ramo epiploico derivato dal ramo pancreatico (ramo epiploico posteriore), che in due soggetti dava origine anche a un'arteria colica accessoria. Questi due ultimi esempi non sono stati considerati equivalenti all'arteria colica sinistra superiore.

In alcuni casi dà origine all'arteria colica sinistra e all'emorroidaria superiore (NA, rettale superiore), sostituendo così in tutto o in parte l'arteria mesenterica inferiore.

L'arteria mesenterica inferiore può originare dalla mesenterica superiore anziché dall'aorta.

L'arteria mesenterica superiore talvolta origina dal tronco celiaco o in comune con esso.

I rami colici e intestinali di questa arteria possono variare considerevolmente per origine e decorso, nonché per il numero di anse primarie e secondarie che formano.

Una variazione rara è la persistenza di un'arteria onfalomesenterica che decorre in prossimità dell'ombelico e fornisce un ramo all'uraco, al fegato attraverso il legamento falciforme oppure al muscolo retto dell'addome, anastomizzandosi con l'arteria epigastrica.

Un numero di suoi rami compreso tra nove e diciotto irrora il digiuno e l'ileo, sebbene sia più comune un'irrorazione affidata a un numero di rami compreso tra undici e quattordici. L'intervallo minimo/massimo per il digiuno e l'ileo va da sei a ventuno rami.
